A sports physical is required in order for students (Grades 6-12) to participate in the athletic program.  A sports physical form may be downloaded for your convenience and must be turned in before a student can play any team sport. 

NOTE:  This form may serve as a school physical however, a school physical form will not suffice as a sports physical.
