New students enrolling at Spring Valley Academy must fill out and sign an Admission Information Sheet and give to the previous school to complete and release all school  records. (academic, behavior, etc.)  This includes the academic transcript of report cards, standardized test records and immunization records. 

Grades 6-12 must also secure three Recommendation Forms to be filled out by at least two teachers and a character reference, i.e., pastor or youth pastor, which can then be mailed or faxed to SVA.

This information is absolutely necessary in order to arrange the entrance interview with administration.
